New Client Intake

Thank you for taking the time to complete this intake to begin working with Sage Movement. Not every question needs answering, however being thorough with your answers is helpful to create a strong starting point in our work together.

All sessions are currently held virtually via Zoom.

You'll receive a calendar invite by email for your sessions.

Prepayment and completion of the intake form and waiver are required at least 24 business hours prior to your first scheduled appointment. If a medical provider letter is required (see the PAR-Q+ section), it must be submitted at least 72 business hours prior to your first appointment.
